If the Pennsylvania Department of Labor and Industry has told you
that you owe an overpayment for PUA, PEUC, or FPUC, and the
overpayment did not result from “fraud” or “fault,” you can request
that the overpayment be “waived” (meaning you don’t have to pay it).
The Pennsylvania Department of Labor and Industry may accept your
request if you demonstrate that repaying the money would cause a
financial hardship to you.
In order to show that repaying the overpayment would cause a
financial hardship, you have to fill out an “Overpayment Waiver
Questionnaire” containing information about the income and expenses
of your household.
This web application will prepare an “Overpayment Waiver
Questionnaire” document for you based on information you provide.
Providing documentation of expenses
After answering questions about your income and expenses, you will
need to provide proof of your expenses. If you are using a
smartphone, you can use your phone’s camera to take pictures of
receipts, statements, etc.
If you are not using a device with a camera, you can upload images
of documents if you have them.
Saving your answers and resuming later
The questionnaire is long, because there are many questions about
your household finances. If you want to take a break, you can go to
the menu in the upper-right corner and press “Come back later.”
This will give you a link you can use to resume the interview later.
You can e-mail the link to yourself or text it to yourself.
If you are using a computer now, you might want to use the “Come
back later” link to switch to a smartphone for purposes of taking
pictures of the documentation of your expenses. We will ask you to
upload documentation at the end of the process.
Submitting your “Overpayment Waiver Questionnaire” to the Department of Labor and Industry
At the end of the questionnaire, if you have uploaded documentation
of your expenses, you will have the option of submitting your
“Overpayment Waiver Questionnaire” directly to the Department of
Labor and Industry through this web app.
In addition:

it to the Department of Labor and Industry yourself.
- You can download it, print it, and mail it to the Department of
Labor and Industry yourself.
